While the industrial and consumer uses of silicone oil are widespread, its most critical and demanding applications are often found in the fields of medicine, pharmaceuticals, and high-purity manufacturing. In these sectors, its unique combination of biocompatibility, chemical inertness, and non-toxicity elevates it from a mere industrial fluid to a material that is essential for human health and technological advancement. One of the most remarkable medical applications of silicone oil is in ophthalmology, where it is used in complex retinal surgery. In severe cases of retinal detachment, particularly those with a lot of scarring or tears, a surgeon will remove the vitreous humor (the gel that fills the eye) and replace it with silicone oil. The oil typically remains in the eye for several months before being surgically removed.

In the broader medical and pharmaceutical industries, silicone oil is used as a highly effective lubricant for medical devices and equipment. Its lubricity and non-toxic nature make it ideal for coating the inside of syringes, allowing for a smooth and consistent injection. It is also used as a lubricant for surgical instruments, catheters, and other medical tubing, where a non-reactive and non-stick surface is essential to prevent contamination and ensure smooth operation. In pharmaceuticals, a small amount of silicone oil may be used to polish tablets and capsules, or to coat the inside of medicine bottles to ensure that every last drop of a liquid medication can be easily poured out.

Finally, silicone oil's exceptional purity and inertness make it an ideal fluid for high-purity industrial applications, such as in the semiconductor manufacturing process. The fabrication of microchips requires an ultra-clean environment, and any contaminants can ruin an entire batch. Silicone oil can be used as a heat transfer fluid or a lubricant in these cleanrooms because it does not contain the impurities found in conventional oils that could outgas and contaminate the sensitive electronics. Its non-reactive nature also means it will not degrade in the presence of the harsh chemicals used in the manufacturing process.
